(II) Restoration — No. 06
All-on-4 and All-on-6
A whole jaw, one plan.
Rebuilding a toothless — or nearly toothless — jaw with a fixed row of teeth on four or six implants.
At a glance
- Surgery
- Usually a few hours per jaw
- Temporary teeth
- Soon after, in suitable cases
- Final bridge
- Usually after 3–6 months
- Anaesthesia
- Local
Durations and visit counts are typical values; they vary from person to person and become clear after an examination.

All-on-4 and All-on-6 carry an entire row of teeth on four or six implants in a toothless jaw. Unlike a removable denture, it does not move, does not cover the palate and never has to be taken out.
At the heart of the method is the strategic angling of the implants. In many cases this makes the best use of the bone that is there, and can reduce the need for extensive bone grafting.
Rebuilding a jaw means rebuilding the lower third of the face, not only the teeth. Lip support, how much of the teeth shows in a smile, and speech are as much a part of the plan as the bone.
Who it is for
- Those who have lost all their teeth
- Those about to lose most of their teeth
- Anyone uncomfortable with a removable denture
- Some patients without enough bone for individual implants
Options
- 01
All-on-4
A fixed bridge on four implants; often chosen where bone is limited.
- 02
All-on-6
A fixed bridge on six implants; spreads the load more widely when the bone allows.
- 03
Temporary and final bridge
Fixed temporary teeth soon after surgery in suitable cases; the final bridge after healing.
The process
Full analysis
General health, bone structure and bite are assessed with 3D imaging.
Design
Tooth position, lip support and smile line are planned; the implants are positioned for this plan.
Surgery
Any extractions and the placement of the implants can be done in the same session.
Temporary teeth
In suitable cases, fixed temporary teeth are fitted shortly after.
Final bridge
Once healing is complete, the final bridge is made and fitted.
Follow-up
Regular check-ups and professional cleaning keep the result.
—The Dent BB approach
We design the face first, then the implants. Lip support, the smile line and speech sit at the centre of the plan; the implants carry those decisions.
Honest expectations
- This is extensive surgery; general health is assessed with great care.
- Fixed temporary teeth on the same day are not possible in every case; it depends on the bone.
- Cleaning the bridge calls for specific care habits.
- Smoking and uncontrolled systemic conditions reduce success.
Aftercare
- A water flosser and dedicated brushes to clean under the bridge
- A soft diet in the first weeks
- Regular check-ups and professional cleaning
- A night guard, if needed
Frequently asked
All-on-4 or All-on-6?
The choice depends on the amount of bone, the shape of the jaw and the bite. Both give a fixed row of teeth.
Will I have teeth on the same day?
If the bone allows, fixed temporary teeth can be fitted on the implants shortly after surgery. The decision follows imaging and examination.
Can the bridge be removed?
Not by you; your dentist can remove it when needed for check-ups and cleaning.
Will my speech change?
There may be a short period of adjustment. Because the bridge does not cover the palate, most people speak more comfortably than with a removable denture.
